Please fill out all pages of the application form below. Completing the first page of the application form is the first step in requesting your your spot in the program, it will not be held until you have paid and spoken with our Continuing Education Advisor to confirm your prerequisite submission, and secure your space in the class.

If the workshop is full, or if the workshop has not yet been scheduled, the form’s completion will be considered placement on our wait list, and we will inform you first when the workshop schedule is set.
